Leading into the second phase of one of the boss fights a cutscene plays of a severely mutilated, lunatic man cutting off his forearm. After his forearm is dismembered his arm is turned into a dragon and is used to attack. This scene is part of a skippable cutscene and can be skipped without losing any story elements.
The player can stab, impale, slash, smash, burn, and blow up enemies with a variety of in-game weapons, and vice versa. By default, these attacks cause blood to splatter out of players and enemies, however, blood can be reduced or turned off entirely.
There is a mechanic known as "hemorrhage buildup", in which certain weapons and abilities can cause enemies to "bleed" after a certain amount of hits. This "bleed" effect is signified by a large blood splatter exploding out of the enemy, and the blood can be seen lingering on surfaces.
Bloody combat is seen throughout but the more gruesome violence is done towards fantasy characters.

The game's brilliant art direction is very successful at portraying a spooky, disquieting world and disturbing character design for monsters. All this makes it creepy and non-suitable for pre-teens.
There are many disturbing and/or shocking moments. One boss cuts off its arm in a skippable cutscene, and another is depicted eating monsters (no blood or gore is shown).
Some of the enemies and creatures feature very disturbing designs. These included a creature with many limbs and a human face inside, a centipede creature, and many other bizzare and disfigured creatures.
A monstrous like man begins to viscerally scream in pain as fire consumes him.
There are abominations with several limbs that crawl around like a spider and have a paralyzed face like it was frozen as the face was screaming.
